[Curriculuminventory] Survey regarding academic levels

Dear colleagues,

We have created a survey about how "Academic Levels" are structured in the curriculum at your school.

For all questions, please refer to the following definition of academic levels (taken from the MedBiquitous Curriculum Inventory Glossary):

Defined temporal stages of training within a program which students complete sequentially. For example, Academic Levels are frequently defined by academic years or phases. In some instances, academic levels may overlap.
