ASPIRE: A Continuous Quality Improvement Model for the Improvement of Patient Population Health

Objectives: At the end of this session, participants will be able to: -Describe a real world collaboration with a commitment to improve patient care by transforming pharmacy practice -Describe how a regional collaboration with a technology partner can scale performance assessment and benchmarking -Identify opportunities to incorporate medication use quality metrics into performance improvement efforts -Identify factors that may facilitate or barricade the successful implementation of a medication adherence intervention Methods: Presenters will describe the innovative and robust model that improves patient population health through the transformation of community pharmacy practice using a cloud-based technology platform. ASPIRE™ includes the use of a web-based platform, powered by CECity and designed to connect stakeholders in a meaningful way to drive continuous performance improvement and lifelong learning. Through the ASPIRE Portal, pharmacists access a performance dashboard that continually monitors adherence measures, allowing the pharmacists and organizations to track performance on a real-time basis. The technology platforms: 1. provide continuous performance reporting against benchmarks and targets to drive continuous improvement; 2. provide access to online components of ASPIRE training program; and 3. provide access to additional educational resources for pharmacists and consumers.
